"Lack of Service Leads to Missing Important Medication"

(as the patient),

The Circle Medical office did not forward my medication request on to my doctor and because of that I ran out of an important medication. The doctor was on vacation but nobody took care of their patients when they were away. Because of this lack of service, I have moved onto another doctor.
