"Foothills Primary Care Network Chronic Pain Program"

About: Calgary Foothills PCN Calgary Foothills PCN / Extended Health Team

(as the patient),

I was referred to the chronic pain program.  My case manager immediately referred me to physiotherapy and offered to make appointments with several specialties to inform and support me with the many questions that arose when I moved to Alberta.  Finding a physician was challenging, and all the supports I had to leave put me in a place of despair which aggravated the pain.  After many opportunities to attend workshops, classes, and meet with experts I was guided through the maze that essentially was blocking me from living my best life.  They worked as a team and I felt my best interests were always kept in mind.  Thank you to a wonderful team.  Your compassion and support increased my resilience to take on the challenges and continue moving forward.  I appreciate you very much.   All the best to the team with thanks!

Thank you for sharing your story and very positive experience with our Extended Health Team Program.

Moving to a new province and leaving behind all of your supports and starting new must have been very difficult and stressful. We are so pleased to know that your case manager was able to work with you and include referrals to a broader health team who specialized in supporting you in the areas that were having an impact on your pain and well-being.

Navigating a new health care environment can definitely be a 'maze'. Knowing the team was attentive to your needs, compassionate, offered many resource options for support, and helped to guide your care with you at the center, is truly wonderful to hear. We are so happy that you have been able to tap into your resilience to continue moving forward on your wellness journey.

I will pass your message along to the team, the program manager, and our PCN leadership. I will also let them know that you mentioned that the cost of parking could be improved.
