"Becoming Healthier"

About: Aspen PCN / Chronic Disease Management

(as a service user),

I have been overweight most of my life and for the past number of years I have been experiencing atrial fibrillation.  The incidence of AFib was increasing and my cardiologist indicated a change in my lifestyle would improve how I feel.  I have tried unsuccessfully many times throughout my life to address my weight and exercise routine.  My cardiologist referred me to the bariatric nurse at Aspen Primary Care network.  Working with a professional on a regular basis has helped me improve my health and make positive changes in my life.

Through providing ideas, resources, encouragement and regular monitoring, my lifestyle changes have lead to me improving my health.
