                               May 1-3rd, 2025 in Edmonton, Alberta 

The Primary Care Strategic Forum is an annual multi-day conference that focuses on primary care transformation in Alberta. This is where primary health care system leaders and multi-disciplinary providers will gather to learn about the important initiatives underway that advance the Patient’s Medical Home in an Integrated Health Neighborhood. 

Not only will we be sponsoring this event, but we will also be introducing the Care Opinion Canada program by presenting OUR story titled:

                   “A New Path Towards Healthcare Improvement”  

Care Opinion Canada is an innovative, interactive online platform designed to transform healthcare by fostering meaningful communication between patients, families, and healthcare providers. It serves as a pathway for all to share personal experiences and offer feedback that can be constructive, transparent, and impactful. We will explore the power of innovation. The platform redefines how we communicate, breaking down traditional barriers and allowing for real-time, interactive engagement. Utilizing the technology of Care Opinion, we enable patients or caregivers to have a voice and advocate for better care while also contributing to broader healthcare improvements. Change management plays a key role in how the platform functions. Through patient feedback, providers are guided in making informed, actionable changes. We support providers in navigating the complexities of change by creating a space where feedback is not only collected but collated to help drive continuous improvement. Additionally, the platform reinforces the importance of team-based care. We facilitate this concept by providing a space where providers hear directly from people living in Alberta about their experiences. This allows care teams to collaborate more effectively, aligning their efforts around shared goals and improving care coordination.  

Patient feedback helps team members understand how their individual roles and actions contribute to the overall patient experience, leading to more cohesive, patient-centered care. Attendees will engage in a storytelling exercise, reflecting on their own care experiences, and will have the opportunity to explore the Care Opinion platform through a live demonstration. They will hear directly from an early adopter about their journey in implementing feedback and the resulting care improvements, providing actionable insights for participants to apply in their own settings. By the end of the session, attendees will be equipped with tools and resources to harness the power of storytelling, whether by submitting a care story, implementing the platform in their practice, or championing patient engagement within their organization.
